5-alpha-reductase, shortened to 5AR, is usually introduced as the enzyme that turns testosterone into dihydrotestosterone, or DHT. That description leaves out most of its biology.
The same enzyme family begins the path from progesterone to 5-alpha-dihydroprogesterone and allopregnanolone. It helps make THDOC from deoxycorticosterone. It acts on several routes that can lead to DHT, and it helps the body break down cortisol, corticosterone, and aldosterone. SRD5A3 also helps make dolichol, a fat-like molecule that cells use to attach sugar tags to new proteins.[S1][S2][S3][S7][S8][S9][S212]
5-alpha metabolism matters across all of these hormone systems. DHT carries a strong androgen signal. Allopregnanolone and THDOC affect GABA-A receptors, which help control how strongly nerve cells respond. The cortisol and aldosterone branches change how the body processes adrenal hormones. Broad inhibition can affect several steroid families at once.[S1][S2][S5][S7]
High blood DHT does not automatically mean that the body made more 5AR. DHT can rise because testosterone rose, because less DHT was removed, because DHT was taken directly, or because testosterone was applied to skin that converts a large share of it before it reaches the blood. A convincing result needs a direct conversion test, a useful product-to-starting-hormone comparison, or a urine measure that shows more steroid taking the 5-alpha path.
The strongest human findings involve thyroid state, recovery from severe under-eating, and one controlled comparison of a high-carbohydrate diet with a very-high-protein diet. Several smaller hormone studies also found a shift. Most supplement and herb studies measured only a final hormone, one tissue, an animal, or cells in a dish. Those results say less about conversion across the human body.
The practical order starts with low energy intake, too little carbohydrate, confirmed thyroid problems, and medically high prolactin. Resistance training has a direct human result inside muscle. Creatine has one positive blood-ratio trial, followed by a later trial that found no change. Direct DHT can start the positive feedback loop described later. It comes last and needs medical monitoring because it also changes the hormone being measured. Most other popular claims stop at animal tissue, cells, a higher starting hormone, or a higher final hormone without proof of faster conversion.
No standard blood test diagnoses “low 5AR” in a healthy adult, and there is no accepted ideal DHT-to-testosterone ratio for everyone. Rare inherited deficiencies are separate medical conditions. Pregnancy, breastfeeding, adolescence, fertility treatment, prostate disease, liver disease, and prescribed hormone use need their own clinical decisions.[S1][S4][S17]
